Allgemeines Überwiegend bei Kindern.

Epidemiologie

93 % im Alter von 1-8 Jahren. 48 % Stadium 4

Prognose

Faktoren: Ausbreitung in der Leber, Lungenmetastasen, Histologie,

Metastasierung

10-20 % der Patienten haben primäre Lungenmetastasen. Die Überlebensrate liegt zwischen 25 und 50 % (5,6).

Staging

TNM (UICC), Stadieneinteilung nach Okuda.

Diagnostik

Sonographie, CT. AFP zwischen 1000 und 1 Million in 83 %, 17 % größer als 1 Million
Morphologie SCU Small cell undifferentiated Undifferenzierte Zellen mit wenig Zytoplasma
unverdächtige Nukleoli
unzusammenhängende Zellen
ohne Tubuli
infiltrieren die Leber, nicht die Gallengänge

Therapie

Operation

Höchste kurative Chance. Nur 1/3 kurativ operabel.

Radiatio

Chemotherapie

Mit einer präoperativen Chemotherapie (Cisplatin und Doxorubicin) kann die Resektabilität und das Gesamtüberleben verbessert werden(4).
